DVD Order Form

Graduation, a once in a lifetime experience!

A DVD of the entire ceremony will be recorded as it is seen on the Palace scoreboard screen, and you have the opportunity to purchase a copy to commemorate this special event. There are two ways to order your DVD:
 

DROP OFF
(cash, checks, or credit card)
Please complete the DVD Order Form, and drop off your $20 per copy payment
at the OCC Bookstore table in the North Entrance of the Palace on the day of Commencement
or at the Cashiers Office or Raiders Store at any campus no later than July 1, 2009.
 

MAIL-IN
(checks only)
You may also mail in your completed order form with your $20 per copy payment to the following address. Requests for the DVD must be postmarked no later than July 2, 2009.
