Checklist for National Registry Application/Examination Process

Candidates:

Applications must be received by SWM in Kalamazoo at least 30 days before exam.  If other exams are opened up, candidates will be notified of those dates. Reservations are confirmed by e-mail or post card.

Application must have an exam reservation form attached with the two separate fee payments.

No personal checks are accepted.

 Complete NREMT Application:

__        Do not fill in location or date of test at top of form.

__        Make sure Felony Statement information is completed.

__        EMT candidates, do not complete Practical Examination Verification on front page, the Regional Coordinator will do that after your practical exam.  No physician signature is needed.

__        Assure all information needed from your program instructor is completed.

            __  CPR Credential must be signed or copy of card attached

            __  Statement of Competency in EMT-Basic Skills must be signed

__        Attach proof of course completion certificate

___     If initial education is greater than 2 years ago, attach proof of continuing education credits or refresher course

__        For Specialist and Paramedic candidates, attach Basic EMT license or Basic NREMT certification.

__        EMT Candidates, complete character reference information.

__        Be sure to sign the Candidate Statement

__        All signatures must be original or signature stamp

            Complete EMS Exam Reservation Form:

__        Identify upcoming exam dates. (Exam list available from SWM Web site).

__        List 3 choices for exam dates on form.

__        Assure phone numbers, e-mail address and other information is provided so that you may be contacted to confirm exam location.

__        Assure payment to NREMT for written exam is attached.  NO personal checks.

__        Assure payment to SWM Systems for practical exam is attached.  NO personal checks.

 If any of the above information is not provided, your application will be returned to you.

Reservation for examination is not made until all information is received by SWM office.
