EMS Student Paperwork and Forms

All student paperwork and forms are in PDF format. You will need to have Adobe Acrobat Reader installed on your computer to view and print the paperwork. You can download Acrobat Reader from Adobe's web site.

All students should read and print a copy of the handbook and the Exposure Control Plan for their records.  The final page of the handbook file as well as the final page of the Exposure Control Plan should be printed, signed, and turned in to the EMS Program office.
